Privacy and parent authorization

JOGA NYC uses the information in this form to review your request, organize the selected soccer program, and communicate with your family. For a player under 18, a parent or legal guardian must complete or authorize this form and provide their own name, email, and phone number. Children under 13 should have a parent complete the form.

How this form uses your information

Depending on the form, we request player identity, age or birth date, soccer background, contact details, and participation needs. Provide only what is needed for this request. Do not include medical records, identification documents, or other sensitive documents in notes or links.

Registration information is handled through JOGA’s form and recordkeeping services. The selected program may involve a named host or club; ask JOGA about recipient-specific information sharing before providing optional player footage or profiles. Registration authorization does not authorize unrelated sponsor marketing or commercial reuse.

Photo and video permission is a separate choice wherever requested. Agreeing to registration or event updates does not by itself authorize promotional email, marketing photography, or nonessential third-party disclosure.

Contact info@joganyc.com to request access, correction, deletion, or withdrawal of a permission. Include the program and a parent contact so JOGA can follow up; do not email identity documents. Additional parental verification may be needed. This acknowledgment alone is not verified parental consent for services that require it.

The goal is not to send every player to every session. It is to contact players when there is a real fit between their profile and the evaluation environment.

That can mean a youth academy, ECNL, NAL, MLS NEXT Academy Division, MLS NEXT Homegrown, university, or professional pathway.

Each future Talent ID will have its own age range, level, pathway, format, and availability. Any date, location, or cost will be shared before you accept an invitation.

Submitting the form does not guarantee an invitation. It puts your profile in front of JOGA for the next suitable opportunity.
